Thursday February 18 & Friday February 19 from 8am-noon CT

Dear Collaborative Communication Workshop participants:

We look forward to meeting you next week at the Collaborative Communication Workshop! To help you define your personal goals and to help the training team learn more about you before the workshop, we ask that you complete the diagnostic form below. The form has four questions, and responses are anonymous. Past participants have found that taking time to better understand their own personal experiences, goals, and needs prior to the workshop makes the time spent in the workshop even more meaningful. To ensure that the training team has adequate time to review the responses, we ask that you complete this diagnostic form no later than Monday February 15 at 5pm CT.

Many participants find it helpful to have a specific situation in mind during the workshop.
